About The Position

We are seeking dedicated and compassionate healthcare professionals with skilled nursing facility (SNF) experience to join our team in providing high-quality care to residents at long-term care and nursing homes.

Requirements

• Previous experience in a skilled nursing facility or long-term care setting is preferred.
• Valid nursing license or certification in the relevant field.
• Strong communication and interpersonal skills.
• Ability to work collaboratively with a multidisciplinary team.
• Compassionate, patient-centered approach to care.
• Flexible and adaptable to changing schedules and environments.

Responsibilities

• Provide direct patient care including administering medications, treatments, and monitoring vital signs.
• Assist residents with activities of daily living, including bathing, grooming, and mobility.
• Ensure a safe and hygienic environment for residents.
• Build relationships with residents and their families to promote a supportive community.
• Document and communicate patient information effectively with the healthcare team.
• Participate in care planning and patient assessment processes.
• Adhere to all health and safety regulations and facility policies.
