About The Position
PASCO/SW Home Health is seeking an Occupational Therapist to provide home health care that helps patients regain independence. The role includes creating personalized treatment plans, teaching adaptive skills, and collaborating with a supportive team. The position offers flexible scheduling and autonomy in patient care. Compensation is based on a per-visit schedule, with a stated rate of $40.00–$57.00.
Requirements
- Be a licensed Occupational Therapist (OT) in the State of Colorado.
- Graduate from an ACOTE-accredited OT program and be eligible for NBCOT certification.
- Have a minimum of 2 years of clinical experience; home health experience is preferred.
- Hold current CPR certification.
- Have a valid driver’s license, insurance, and reliable transportation.

Responsibilities
- Develop and implement individualized care plans.
- Provide therapy services in accordance with physicians’ orders.
- Assess patients’ functional status and recommend adaptive equipment.
- Teach new skills or retrain patients in daily activities.
- Maintain accurate documentation and participate in case conferences.
