About The Position
Uplift HomeCare, LLC is seeking an Occupational Therapist to provide home health services in Odessa, Florida, and surrounding communities. The role focuses on helping seniors and veterans improve daily functioning, regain independence, and remain safely in their homes through compassionate, individualized care. The position offers part-time work, flexible scheduling, professional independence, and compensation of up to $100 per visit. Start may be immediate once onboarding and credentialing requirements are complete.
Requirements
- Graduate of an accredited Occupational Therapy program.
- Active Florida Occupational Therapist license.
- Current CPR certification.
- Required AHCA Level II background screening.
- Strong clinical judgment, communication, and organizational skills.
- Ability to work independently and manage field visits effectively.
- Valid driver's license, current auto insurance, and reliable transportation.
Nice To Haves
- Home health experience preferred.
Responsibilities
- Evaluate patients in their homes and assess functional abilities and individual needs.
- Develop individualized treatment plans and measurable goals.
- Provide therapeutic interventions to improve activities of daily living, safety, and independence.
- Educate patients and caregivers on home exercises, adaptive techniques, and fall prevention.
- Monitor patient progress and adjust treatment plans as clinically appropriate.
- Complete accurate, timely, and compliant clinical documentation.
- Collaborate with the interdisciplinary care team to support positive patient outcomes.
