About The Position

Agape Hospice & Homecare is hiring a full-time Occupational Therapist to provide home health services in Pinal County. The role offers a Monday–Friday schedule and flexible scheduling intended to support work-life balance. The Occupational Therapist develops individualized treatment plans, helps patients regain independence and improve daily living, and works autonomously with support from a care team. The employer is an independent operating subsidiary within The Pennant Group, Inc.

Requirements

• Active Occupational Therapist license in the State of Arizona.
• Graduate of an ACOTE-accredited occupational therapy program and eligible for NBCOT certification.
• Minimum of 2 years of clinical experience; home health experience is preferred.
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services according to physician orders.
• Assess patients’ functional status and recommend adaptive equipment.
• Teach patients new skills or retrain them in activities of daily living.
• Maintain accurate documentation.
• Participate in case conferences.
