About The Position

Sage Group HealthCare Staffing is seeking an Occupational Therapist for a 13-week travel/contract position in Marshfield, WI. This role involves providing Occupational Therapy services to patients in their homes, utilizing skills in evaluation and treatment to maximize patient independence. The therapist will plan and prepare treatment plans based on patient data, provide in-home OT treatment programs, and help patients and their families maintain dignity and independence. The position requires driving to patient homes in the Marshfield, WI and surrounding areas on a daily basis.

Requirements

Current Occupational Therapist experience: 1-2 years - Required
Recent/Current Home Health OT experience: 12 months within the last 3 years - Required
Must have valid Driver’s License and Personal Auto Insurance
Bachelor of Science degree in Occupational Therapy or Similar – Required
Current State License as Occupational Therapist or Compact if applicable - Required
BLS Certification from AHA or American Red Cross - Required

Nice To Haves

Home Care Home Base (HCHB) charting experience - Preferred

Responsibilities

Provide Occupational Therapy services to patients/clients at their homes.
Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential.
Plans and prepares treatment plan based on evaluation of patient data.
Provide in-home OT treatment programs to assist patients in restoring function.
Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home.
Drive to patient homes in the assigned territories on daily basis.
