About The Position
Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, and surrounding communities. This is a PRN opportunity that offers flexibility, professional independence, and the opportunity to make a meaningful impact every day. As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence they need to safely perform everyday activities while remaining in the comfort of their own homes. You'll have the autonomy to create personalized treatment plans, build meaningful relationships, and celebrate every milestone alongside your patients. This role emphasizes one-on-one therapy that creates lasting results, without crowded therapy gyms, rushed appointments, or assembly-line patient care.
Requirements
- Active Virginia Occupational Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Interpersonal Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Complete occupational therapy evaluations, reassessments, and discharge visits
- Develop individualized treatment plans based on patient goals and functional needs
- Provide skilled occupational therapy services in patients' homes
- Assess home environments and recommend adaptive equipment and safety modifications
- Educate patients and caregivers on therapeutic techniques, energy conservation, home safety, and activities of daily living
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
