About The Position

Putnam Nursing & Rehabilitation Center is a 160-bed Skilled Nursing Facility hiring a PRN/Per-Diem Occupational Therapist, (OT) to join our growing rehabilitation team. Our PRN Occupational Therapist, OT will work closely with other rehabilitation staff and the interdisciplinary care team to help residents improve activities of daily living (ADLs), strength, and independence. Putnam Nursing & Rehabilitation Center is committed to providing compassionate, high-quality care through an interdisciplinary team approach. Requirements

  • Must possess a current license and registration to practice as a Occupational Therapist, OT in the State of NY.
  • Must possess current malpractice insurance, license in good standing.
  • Dedicated to providing extraordinary compassionate care as a Occupational Therapist, OT.
  • Energetic, enthusiastic, committed, & passionate about quality care.
  • Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents.
  • Patience, a cheerful disposition, & willingness to assist any residents & co-workers.
  • Excellent communication, customer service, & organization skills.
  • Trusted to work independently & in a team environment.
  • Able to work well under pressure, flexible, & will work harmoniously with all staff.

Responsibilities

  • Licensed Occupational Therapist (OT) providing PRN (as-needed) rehabilitation services in the Facility.
  • Evaluating and treating geriatric patients with complex medical, cognitive, and functional needs.
  • Developing individualized treatment plans to improve mobility, daily living skills, and overall functional independence.
  • Collaborating with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists to deliver coordinated, patient-centered care.
  • Skilled in adaptive equipment training, fall prevention strategies, and patient/caregiver education.
  • Flexible and responsive, providing high-quality care across variable schedules and patient acuity levels.
