About The Position

Vertis Therapy is looking for a passionate PRN Occupational Therapist who loves working with elders for our Assisted Livings with memory care in the South Bend and Mishawaka, IN area. Vertis Therapy is a therapist owned company that focuses on providing quality, patient centered care. We are especially interested in candidates with lymphedema certification or specialized experience in lymphedema management.

Requirements

• Degree in Occupational Therapy from an accredited institution.
• Valid Occupational Therapist state license or license eligibility
• Strong clinical assessment skills.
• Proficient in developing and implementing effective treatment plans.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team environment.
• Ability to lift and assist clients as needed.
• Must be able to stand for extended periods and perform physical tasks related to therapy.
• Computer proficiency to document in electronic medical record platform.

Nice To Haves

• Current registration with the American Occupational Therapy Certification Board.
• Lymphedema certification or specialized experience in lymphedema management.

Responsibilities

• Evaluate residents' functional abilities.
• Establish and execute a plan of care based on individual goals to improve resident's independence and quality of life.
• Complete timely and accurate documentation.
• Provide appropriate guidance and supervision to Certified Occupational Therapy Assistants.
• Participate or lead team development meetings.
• Regularly lead community wide wellness programs and events.
