About The Position

Discovery at Home is seeking an Occupational Therapist in Charlotte County, FL. This role involves home visits with potential for some caseload within assisted and independent living communities. The patient population is primarily geriatric, with most cases under Medicare Part A Home Health, along with some Part B. Part-time and PRN candidates will be considered, up to 3 days per week.

Requirements

Graduation from an Occupational Therapy program.
State license as an Occupational Therapist (eligible for FL state licensure).
Valid driver’s license and reliable transportation.
Current CPR certification.
Minimum of 1 year of prior home care experience.
Must complete a Level 2 background screening in accordance with Florida AHCA requirements, including fingerprinting and background screening through the AHCA Clearinghouse.

Responsibilities

Primarily work home visits with the potential for some caseload within assisted and independent living communities.
Manage caseloads and work schedules, with clinicians determining their own schedules.
Provide care to a primarily geriatric patient population.
