About The Position

Total Care Therapy (TCT) is seeking a passionate Occupational Therapist (OT) with leadership experience or aspirations to join their expanding team. This role combines hands-on patient care with leadership responsibilities, overseeing the daily operations of the rehabilitation department in assisted living and independent living communities. The OT/Director of Rehabilitation will serve as the main liaison between facility leadership and TCT leadership, ensuring exceptional patient outcomes, operational excellence, and regulatory compliance. This position involves providing skilled occupational therapy services, supervising COTAs, managing administrative tasks, driving referrals, and fostering a collaborative environment.

Requirements

  • Current State Occupational Therapist (OT) License
  • Current CPR/BLS Certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong communication, organizational, and leadership skills

Nice To Haves

  • Experience in geriatrics, home health, assisted living, or skilled nursing
  • Previous leadership, supervisory, or Director of Rehabilitation experience (leadership potential will be considered)

Responsibilities

  • Provide skilled occupational therapy services according to established plans of care.
  • Deliver therapeutic activities and interventions that improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, coordination, balance, cognition, and functional independence.
  • Monitor patient progress and modify treatment plans as needed to achieve functional goals.
  • Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
  • Complete timely and accurate documentation using the electronic medical record (EMR).
  • Educate residents and caregivers on adaptive equipment, home safety, energy conservation techniques, fall prevention, and strategies to maximize independence.
  • Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
  • Maintain a productive clinical caseload while delivering compassionate, high-quality therapy services.
  • Act as the main liaison between leadership and the facility.
  • Oversee staff performance and support professional development.
  • Manage administrative and operational tasks across therapy services.
  • Coordinate meetings, training, and events for staff and residents.
  • Maintain communication with residents, families, and leadership.
  • Drive referrals for all disciplines
