About The Position

Total Care Therapy (TCT) is seeking a compassionate and motivated Physical Therapist (PT) to join their growing team. This role involves evaluating, developing, and implementing individualized treatment plans to help residents in assisted living and independent living communities improve mobility, restore function, reduce pain, and maintain independence. The position also includes Director of Rehabilitation responsibilities, acting as a liaison between leadership and the facility, overseeing staff, managing administrative tasks, coordinating meetings and training, maintaining communication with residents, families, and leadership, and driving referrals for all disciplines. Requirements

  • Current State Physical Therapist (PT) License
  • Current CPR/BLS Certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong communication, organizational, and leadership skills

Nice To Haves

  • Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
  • Previous leadership, supervisory, or Director of Rehabilitation experience preferred, but leadership potential will be considered

Responsibilities

  • Provide skilled physical therapy services according to established plans of care.
  • Deliver therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions.
  • Monitor patient progress and modify treatment plans as needed to achieve functional goals.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
  • Complete timely and accurate documentation using the electronic medical record (EMR).
  • Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
  • Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
  • Maintain a productive clinical caseload while delivering compassionate, high-quality therapy services.
  • Act as the main liaison between leadership and the facility.
  • Oversee staff performance and support professional development.
  • Manage administrative and operational tasks across therapy services.
  • Coordinate meetings, training, and events for staff and residents.
  • Maintain communication with residents, families, and leadership.
  • Drive referrals for all disciplines
