About The Position

Synergy Care is seeking a full-time Certified Occupational Therapy Assistant (COTA) for an outpatient clinic serving an Assisted Living/Independent Living (AL/IL) community at Franklin Park in Boerne, TX. Join our team! We are looking for a compassionate, collaborative clinician who is dedicated to providing exceptional patient care and making a positive difference in the lives of those we serve.

Requirements

• Graduate of an ACOTE-accredited Occupational Therapy Assistant program (or graduating within 6 months)
• Current NBCOT certification (COTA) required
• Active Texas OTA license (or eligible for licensure)
• Strong communication and teamwork skills
• Commitment to patient-centered care

Responsibilities

• Implement treatment plans established by the Occupational Therapist
• Provide skilled occupational therapy treatments to residents
• Document patient progress and communicate with the therapy team
• Assist with discharge planning and patient education
• Participate in interdisciplinary case management
• Deliver high-quality therapy services in accordance with state regulations and Synergy Care standards
• Represent Synergy Care's commitment to excellence in every patient and facility interaction
