Requirements

Earned an MD or DO degree.
Completed an OBGYN residency.
Board eligible or board certified.
Have an unrestricted Arizona License and DEA.
Approval to hospital medical staff for OB deliveries and GYN Surgeries.
Preference for or experience in an FQHC, CHC patient population.
Have a Level One Fingerprint Clearance card.
Obtain and maintain vaccination status (i.e., as recommended by CDC and/or other public health agencies) to include an Influenza vaccination.
Undergo drug testing prior to employment and be subject to post-accident, reasonable suspicion, return to duty and follow up drug and alcohol testing.
