About The Position

The Clinical Staff Pharmacist assists the Department Director in providing specialized pharmaceutical services, including advising physicians on drug usage and control within assigned hospital locations. This role also involves participating in the development and delivery of clinical services and continuing education programs for Pharmacy, Patient Care Services, Medical staff, and students/residents.

Requirements

  • Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree or Pharm.D.
  • Currently licensed to practice pharmacy in the state of Ohio.
  • Hospital/clinical pharmacy practice experience required.
  • Ability to work independently with minimal direct supervision.
  • Ability to work cooperatively with pharmacy staff, hospital staff, and physicians.
  • Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
  • Ability to set priorities, make critical decisions and respond quickly to emergency requests.
  • Foster a team environment by providing orientation and training to new team members.
  • Punctual attendance at assigned work location is required.
  • Ability to be mobile.
  • Ability to communicate effectively (verbal and written).
  • Ability to comply with OSHA regulations, USP 797/800 standards, CDC standards, and transmission-based precaution recommendations and to utilize proper personal protective equipment.
  • Ability to comply with provisions of applicable S.D.S. forms.

Responsibilities

  • Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate medications and doses.
  • Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions; ensure compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.
  • Mentor pharmacists in training including, but not limited to, Pharmacy Practice Students and Residents, Medical Students and Residents.
  • Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and HFAP Standards.
  • Conduct and evaluate medication histories, assess compliance and suggest modifications so as to achieve desired outcomes.
  • Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.
  • Serve on the hospitals numerous patient safety and quality committees.
  • Conduct and participate in research including, but not limited to, performing as the primary investigator or co-investigator for research programs which will impact the delivery of quality care or examine the pharmacoeconomic impact of providing care to the patients in the Firelands Health System.
  • Perform other related duties incidental to the work described herein.
  • Prepare and dispense drug orders per physician request according to established policies, procedures, and protocols.
  • Interpret drug orders (verbal and written) and transcribe to computerized patient medication profiles accurately. Maintain accurate, complete pharmacy patient medication.
  • Compound and dispense pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products accurately.
  • Ensure safe, appropriate, cost-effective drug therapies for patients according to established policies, procedures, and protocols.
  • Work independently with minimal supervision. Organize and prioritize work assignments. Ensure pharmacy services are provided in a timely manner.
