Corewell Health Lakeland is a not-for-profit, community-owned health system dedicated to improving health, inspiring hope, and saving lives through compassionate care and wellness encouragement. The system includes three hospitals, an outpatient surgery center, long-term care, and hospice care with over 56 access points in Southwest Michigan. A recent expansion includes a new facility in Niles, Michigan, housing primary and specialty care services to serve the community's health needs. The Clinical Pharmacist will provide daily patient-focused clinical pharmacy services to inpatients and outpatients of all age groups within the Lakeland Regional Health System. This includes medication dosing, chart review, patient education, floor rounds, and physician intervention. The role also involves committee participation, special reports, and rotation through the distributive pharmacy. Responsibilities include order entry, performing all functions of a staff pharmacist, and completing medication reconciliation duties. Additionally, the pharmacist will provide education to patients, nurses, the community, and physicians, participate in staff development, and provide clinical training to Pharm. D. students. Other duties include performing daily chart review, floor rounds, physician intervention, providing long-term care nursing home consultation, participating in investigational drug protocols, and accurately inputting interventions and adverse drug reactions (ADRs). The role also involves IV compounding and verification, adhering to quality control procedures, and a commitment to continuous improvement of the patient experience, including patient safety, quality of care, and excellent service, while adhering to the organization’s published values.
