About The Position
This role involves assessing and reassessing older adults for the Home Delivered Meal program, tracking data for grant requirements, referring clients and caregivers to other services, coordinating meal delivery with staff, and communicating effectively with families, caregivers, and older adults. Other duties may be assigned.
Requirements
- Good driving record
- Basic computer skills

Responsibilities
- Set appointments to assess and reassess older adults for the Home Delivered Meal program.
- Track all data in computer system as necessary for grant requirements.
- Refer clients and caregivers to other services and agencies as necessary.
- Work with staff to set up meal delivery.
- Communicate effectively with families, caregivers and older adults.
- Perform other duties as assigned.
