About The Position

This is a professional position responsible for assessing the needs of Meals on Wheels (MOW) recipients. This position acts as an advocate for individuals receiving Meals on Wheels services, assists with all aspects of the MOW program, and may provide other support to the nutrition department as needed.

Requirements

  • Must pass a Florida Level 2 background check.
  • Bachelor’s Degree in social work, psychology, sociology, nursing, gerontology or related field. Associates Degree with related experience may be considered.
  • Ability to read and write, carry out written and verbal instructions, and complete required reports.
  • Fluent in both verbal and written English and Spanish preferred.
  • 701B Assessment certified by the Department of Elder Affairs (to be completed as part of onboarding process upon hire).
  • Minimum of one-year experience in completing intake screenings, assessments and interacting with seniors is preferred.
  • Must have a valid drivers license, three years or more of driving experience with a reliable vehicle.
  • Excellent interpersonal, communication and organizational skills and the ability to work with individual of various and diverse backgrounds.
  • Knowledgeable and competent to perform all essential functions.
  • Ability to visually and audibly perform all essential functions.
  • Must be able to sit for long periods of time. Sufficient mobility and strength to move about the facility and the community, have corrected vision and hearing to the normal range.
  • This position may require occasional bending and lifting up to 25 pounds, and subject to any other specific physical requirements and working conditions as listed in the job duties.
  • Sufficient skills to be able to read and write effectively.
  • Assumes responsibility for professional growth and development.

Responsibilities

  • Assess client needs and determine eligibility for nutrition services as well as the appropriate funding source with direction from the direct supervisor or designated team member. Once approved for services, coordinate with the MOW team to ensure appropriate delivery route assignment. Coordinates nutrition services for all MOW program recipients.
  • Makes appropriate referrals for any identified and needed Home and Community-Based Services with direction from the direct supervisor or designated team member.
  • Conducts assessments with individuals referred for MOW services, to include annual reassessments and additional assessments when a recipient reports and experiences any significant changes to their health or situation which impacts their nutrition and other needs. Responsible for maintaining a monthly calendar to ensure recipient annual reassessments are completed within program requirements and compliance standards.
  • Support the MOW nutrition team by receiving and organizing MOW service authorization requests from the case management department or other funded program case managers. Assist the nutrition department with maintaining accurate MOW recipient and services data, to include accurate recipient information for database(s) used by the MOW and nutrition department.
  • Support the MOW team with in maintaining the waitlist for MOW nutrition services.
  • Assist the department in providing outreach and educate the public about services available and provided by the MOW and nutrition services department, as well as other services provided by the Osceola Council on Aging.
  • Assist the nutrition team with special and holiday events for the MOW program, other nutrition department special events, and assist with MOW fundraising activities and events.
  • Assist with the delivering Home Delivered Meals to MOW recipients on any designated meal route, and my assist with taking meal temperatures for program compliance, as needed.
  • Follow all policies and procedures of the Osceola Council on Aging.
  • Other duties as assigned, to include assisting in times of disaster or emergency as needed.
