The patient navigator supports the organization’s commitment to value-based, patient-centered health care by helping care teams improve quality performance and patient engagement across the Central New York Network. The role uses a population-health and data-driven approach to identify patients and providers who may benefit from support, encourage preventive and chronic care management, improve health-maintenance documentation in the medical record, and foster strong patient-provider relationships. The coordinator facilitates follow-up appointments and preventive screenings and works with primary care staff to help patients adhere to established medical plans. Key focus areas include annual wellness and follow-up visits, pre-visit planning, closing quality gaps and updating electronic health records, follow-up for patients with poorly controlled A1c or hypertension, preventive cancer screenings, quality reporting and supplemental data abstraction, and clinical coding documentation for high-risk populations. The position reports to the System Population Health Performance Manager and collaborates with primary care teams. It has no staff-supervision responsibility and serves as a technical expert to a group of peers. Pay range is $19.20–$28.80, based on experience and location.
