We are seeking a dedicated RN to oversee quality improvement in our Residential program, supporting individuals with developmental and/or intellectual disabilities. Requirements

  • Current NYS Registered Nurse license
  • Valid NYS driver’s license
  • Ability to participate in on-call rotation

Nice To Haves

  • 5+ years of nursing experience
  • 1 to 2 years related OPWDD experience and/or training
  • Working knowledge of OPWDD regulations
  • Working knowledge of AMAP guidelines
  • Working knowledge of medication administration practices
  • Working knowledge of PONS development
  • Working knowledge of documentation standards

Responsibilities

  • Provide broad clinical support across UCP’s residential programs.
  • Strengthen regulatory compliance.
  • Support continuity of care.
  • Assist with coverage needs across the agency.
  • Conduct scheduled and unscheduled chart reviews and audits for all residential sites to ensure compliance with OPWDD and agency standards.
  • Identify documentation gaps and follow up with residence nurses and supervisors to correct issues promptly.
  • Assist with the creation, revision, and training of PONS for individuals across programs.
  • Reinforce and support implementation of nursing policies and procedures, escalating concerns when required.
  • Monitor and support utilization of electronic MAR and health records to ensure regulatory compliance.
  • Complete AMAP pourings as assigned and ensure medications are prepared accurately and in compliance with agency protocols.
  • Provide coaching to AMAP staff when patterns of medication errors or knowledge gaps are identified.
  • Provide RN coverage for residences with vacant nursing roles, maintaining continuity of care, medical follow-up, and required documentation.
  • Support residential teams during transitions, complex medical events, or surveys and survey preparation.
  • Participate in the nursing on-call rotation, providing timely and appropriate guidance to DSPs, supervisors, and residence nurses.
  • Document all calls in accordance with agency standards and ensure follow-up occurs within the required timeframe.
  • Deliver training and coaching to DSPs, AMAP personnel, and residential leadership as assigned.
  • Support corrective action and re-training efforts following incidents, audits, or survey findings.
