About The Position

The INN Between is Utah’s only medical respite and end-of-life care facility for individuals experiencing homelessness. We provide a safe, supportive, and dignified environment where residents receive compassionate care, stability, and support during critical health transitions, including serious illness, recovery, and end-of-life care. This position is intentionally designed with flexibility and may be structured as a Director of Nursing, Nurse Manager/Lead Nurse, or Evening & On-Call Nurse, depending on the candidate’s licensure, experience, leadership abilities, and organizational fit. The ideal candidate will be grounded in trauma-informed care, comfortable working with individuals experiencing homelessness, and able to balance clinical judgment with compassion, dignity, and respect. This role may include direct resident assessment, care planning, staff support, medical respite coordination, regulatory compliance, and/or nursing leadership. The final scope, schedule, title, and compensation will be determined based on the selected candidate’s qualifications and the needs of the organization.

Requirements

  • Current LPN or RN license in good standing with the State of Utah.
  • RN strongly preferred for Director of Nursing-level responsibilities.
  • Nursing experience in assisted living, hospice, home health, medical respite, behavioral health, public health, or similar settings preferred.
  • Experience working with vulnerable populations, including individuals experiencing homelessness, preferred.
  • Strong clinical assessment, critical thinking, communication, and documentation skills.
  • Ability to remain calm under pressure and exercise sound judgment during emergencies.
  • Ability to work collaboratively with interdisciplinary teams, external providers, and non-clinical staff.
  • Commitment to trauma-informed, harm-reduction-oriented, compassionate care.
  • Leadership experience preferred for candidates seeking Director of Nursing or Nurse Manager-level responsibilities.
  • Ability to work independently while maintaining appropriate communication with leadership and team members.
  • Health evaluation, including TB test and review of immunizations.
  • Must be current on flu and COVID-19 vaccination, including booster, unless an approved exemption applies.

Responsibilities

  • Respond to resident health concerns and provide clinical assessment, guidance, and follow-up.
  • Conduct medical respite assessments and help develop, implement, and update individualized medical respite care plans.
  • Monitor and evaluate changes in resident condition and communicate concerns to appropriate internal staff and external providers.
  • Support care that aligns with Utah Department of Health regulations, Assisted Living Facility requirements, and the Utah Nurse Practice Act.
  • Assist with Service Plans and ensure resident care reflects dignity, safety, and individualized needs.
  • Provide calm, compassionate clinical judgment during emergencies, crises, and end-of-life situations.
  • Collaborate with the medical respite team to integrate medical, behavioral health, hospice, home health, and social service supports.
  • Coordinate with hospitals, clinics, hospice agencies, home health providers, pharmacies, and other partners to support continuity of care.
  • Participate in care conferences, discharge planning, and quality improvement efforts.
  • Support residents in accessing ancillary, therapeutic, and community-based services.
  • Help ensure communication between clinical staff, resident care staff, case management, and leadership is timely and effective.
  • Provide clinical guidance and mentorship to CNAs, Resident Advocates, Death Doulas, medical respite volunteers, and other care team members.
  • Serve as a resource for staff during resident health changes, behavioral health concerns, medication questions, and emergency situations.
  • Support adherence to medication management, documentation, infection control, and emergency procedures.
  • Depending on role level, assist with staff training, competency review, clinical workflows, and performance support.
  • Promote a collaborative, respectful, trauma-informed team culture.
  • Provide oversight and support for nursing systems, clinical documentation, medication management, and resident care standards.
  • Help ensure compliance with HIPAA, Assisted Living regulations, internal policies, and applicable nursing standards.
  • Review incidents, medication errors, changes in condition, and clinical concerns, ensuring appropriate reporting and follow-up.
  • Partner with the Executive Director and Administrator to strengthen clinical operations and resident care practices.
  • Support survey readiness, policy implementation, quality assurance, and continuous improvement.
  • Provide onsite nursing support during evening hours, approximately 2:00 p.m. to 8:00 p.m., or another mutually agreed-upon schedule.
  • Provide overnight on-call support to answer staff questions, assess urgency, guide decision-making, and determine when outside medical intervention is needed.
  • Support CNAs and Resident Advocates in responding appropriately to resident needs after regular business hours.
  • Complete timely, accurate documentation in the EMR and other required systems.
  • Document assessments, care plan updates, interventions, provider communication, and follow-up actions.
  • Report incidents, medication errors, significant changes in condition, and other concerns to appropriate leadership.
  • Maintain confidentiality and comply with HIPAA and organizational documentation standards.
