About The Position

To provide personal care and comfort services to terminally ill patients, regardless of their age, and perform related homemaker tasks in the patient/family residences or Cottingham Hospice House, in accordance with the plan of care, as assigned and supervised by the registered nurse.

Requirements

  • Completion of a nursing assistant or patient care tech program.
  • CNA - NURSING ASSISTANT - NCNA certification.
  • SCDL - SC DRIVERS LICENSE - LSCD.
  • Basic computer skills including word processing and data entry.
  • Knowledge of office equipment (fax/copier).
  • Mathematical skills.
  • Proof of auto liability coverage.
  • Willingness to travel and have reliable personal transportation.

Nice To Haves

  • Participates in bereavement care by attending visitations/funerals.
  • Is knowledgeable in the issues related to death and the normal grieving process and is able to be supportive of patient and family.
  • Participates in team conference and IDG as requested to discuss patient and caregiving issues.

Responsibilities

  • Provide personal care and comfort to patients, including bathing, mouth care, hair care, nail care, shaving, dressing, ambulation assistance, positioning, range of motion exercises, elimination assistance, and feeding.
  • Monitor patient's condition by measuring and recording vital signs, monitoring intake and output, observing and reporting changes in skin condition, monitoring non-sterile dressings, reporting difficulties with elimination, reporting changes in mental status, observing and reporting patient compliance with safety measures, and reporting changes in physical strength, endurance, or coordination.
  • Perform basic housekeeping duties to promote hygiene, comfort, and safety, including changing linens, laundering, washing dishes, cleaning kitchen counters, emptying refrigerators, organizing food and supplies, removing clutter, cleaning and sanitizing durable medical equipment and bathrooms, and running errands.
  • Support general departmental needs by being familiar with policies and procedures, complying with hospice standards of care, participating in QI data review, managing agency resources efficiently, monitoring medical supply utilization, assisting with storeroom organization and cleaning, and assisting with office duties.
