About The Position

To provide personal care and comfort services to terminally ill patients, regardless of their age, and perform related homemaker tasks in the patient/family residences or Cottingham Hospice House, in accordance with the plan of care, as assigned and supervised by the registered nurse.

Requirements

  • Completion of a nursing assistant or patient care tech program.
  • CNA - NURSING ASSISTANT - NCNA certification.
  • SCDL - SC DRIVERS LICENSE - LSCD.
  • Basic computer skills including word processing and data entry.
  • Knowledge of office equipment (fax/copier).
  • Mathematical skills.
  • Proof of auto liability coverage.
  • Willingness to travel and have reliable personal transportation.

Responsibilities

  • Provide personal care and comfort to patients, including bathing, mouth care, hair care, nail and foot care, shaving, dressing, and assisting with ambulation and transfers.
  • Turn and position patients, and assist with range of motion exercises.
  • Facilitate elimination by assisting patients with bedpans, urinals, catheters, or ostomy appliances.
  • Facilitate nutrition by preparing meals, feeding patients, and offering fluids.
  • Participate in bereavement care by attending visitations/funerals.
  • Monitor patient's condition by measuring and recording vital signs, measuring intake and output, observing and reporting changes in skin condition, monitoring non-sterile dressings, reporting difficulties with elimination, and reporting changes in mental status.
  • Utilize communication and listening skills to promote patient comfort and encourage expression of needs or preferences.
  • Observe and report patient compliance with safety measures.
  • Participate in team conferences and IDG meetings to discuss patient and caregiving issues.
  • Report any changes in patient's physical strength, endurance, or coordination.
  • Be knowledgeable in issues related to death and the normal grieving process and be supportive of patient and family.
  • Perform basic housekeeping duties to promote hygiene, comfort, and safety, including changing linens, laundering, washing dishes, cleaning kitchen counters, emptying out-of-date food, organizing food and supplies, delivering medical supplies, removing clutter, cleaning and sanitizing durable medical equipment and bathrooms.
  • Run errands for caregiver/patient as assigned by RN.
  • Support general department needs by being familiar with department policies and procedures, complying with hospice standards of care, participating in QI data review, managing agency resources efficiently, monitoring and maintaining medical supplies, assisting in organizing and cleaning the storeroom, and assisting with office duties as requested.
