About The Position

To provide personal care and comfort services to terminally ill patients, regardless of their age, and perform related homemaker tasks in the patient/family residences or Cottingham Hospice House, in accordance with the plan of care, as assigned and supervised by the registered nurse.

Requirements

Completion of a nursing assistant or patient care tech program.
CNA - NURSING ASSISTANT - NCNA certification.
SCDL - SC DRIVERS LICENSE - LSCD.
Basic computer skills including word processing and data entry.
Knowledge of office equipment (fax/copier).
Mathematical skills.
Proof of auto liability coverage.
Willingness to travel and have reliable personal transportation.

Responsibilities

Provide personal care and comfort to patients, including bathing, mouth care, hair care, nail/foot care, shaving, dressing, ambulation assistance, positioning, range of motion exercises, and elimination assistance.
Facilitate patient nutrition by preparing meals, feeding patients, and offering fluids.
Model competent personal care techniques and communication skills for caregivers.
Perform all personal care tasks for all ages/developmental stages.
Participate in bereavement care by attending visitations/funerals.
Monitor patient's condition and report any changes to the nurse promptly.
Measure and record vital signs (temperature, pulse, respiration, blood pressure, weight).
Measure intake and output (I&O).
Observe and report changes in skin color or any skin tears.
Monitor the condition of non-sterile dressings and change them with RN direction.
Report any difficulties with elimination.
Report any changes in patient's mental status (ability to communicate, level of confusion, aggressive/agitated behavior).
Utilize communication and listening skills to promote patient comfort and encourage expression of needs or preferences.
Observe and report patient compliance with safety measures.
Participate in team conferences and IDG meetings to discuss patient and caregiving issues.
Report any changes in patient's physical strength, endurance, or coordination that would alter care delivery.
Be knowledgeable in issues related to death and the normal grieving process and be supportive of patient and family.
Perform basic housekeeping duties to promote hygiene, comfort, and safety for the patient.
Change linen and bed clothes; launder linen, towels, and clothing (in home).
Wash dishes and utensils; clean and sanitize kitchen counters.
Empty out-of-date food from the refrigerator.
Organize food and supplies; deliver medical supplies as requested.
Work with caregivers to remove unnecessary clutter from the patient's environment to reduce fall risk.
Clean and sanitize durable medical equipment.
Clean and sanitize bathroom areas.
Run errands for caregiver/patient as assigned by RN.
Maintain familiarity with department policies and procedures and demonstrate compliance with hospice standards of care, including infection control and bloodborne pathogens.
Participate in review of QI data and make recommendations for improvement.
Manage agency resources by demonstrating reasonable and efficient use of time, mileage, visit schedules, and pre-visit activity.
Monitor utilization of medical supplies and maintain adequate supply in the home to meet patient needs.
Assist in organizing and cleaning the storeroom.
Assist with office duties (filing, telephone, errands) as requested.
