About The Position

Collage Nursing and Homecare Partners is seeking a nurse to provide daily skilled visits in clients’ homes in Cumming, Georgia. Visits may include care at 6 a.m. and 6 p.m., insulin administration, and weekly medication setup. Pay is $85 per visit, with mileage paid after 30 miles. The role involves providing care in accordance with the patient’s nursing care plan and the RN’s and physician’s plan of care, while supporting patients with daily living activities and self-care.

Requirements

One year of experience as an LPN in a home care setting.
Current state license as an LPN.
Graduate of an accredited practical nursing program.
Current CPR certification.
Proof of a current negative TB test.
Ability to work a flexible schedule and travel locally.
Excellent interpersonal skills.
Valid driver’s license and reliable transportation.

Responsibilities

Participate in planning and coordinating total patient care with the RN and physician’s plan of care, and follow each patient’s nursing care plan.
Accurately report and record the patient’s condition and care, including signs and symptoms that may indicate a change.
Prepare clinical and progress notes and summaries.
Assist patients with activities of daily living while teaching appropriate self-care techniques.
Provide and maintain a safe environment for the patient.
Assist the physician and Registered Nurse in complex nursing situations.
Follow universal precautions when providing patient care.
Follow emergency procedures in the event of a catastrophic event, accident, injury, or significant change in the patient’s condition.
