About The Position

Collage Nursing and Homecare Partners is seeking an LPN to provide daily skilled nursing visits in clients’ homes in Cumming, Georgia. Visit tasks include visits around 6 a.m. and 6 p.m., insulin administration, and weekly medication setup. Compensation is $85 per visit, with mileage provided after 30 miles. The position offers flexible scheduling, free training classes to help meet state CEU requirements, and personal and professional growth opportunities. The nurse participates in planning and coordinating patient care with the RN and physician, follows each patient’s nursing care plan, records patient conditions and care, prepares clinical notes and summaries, assists with daily living and self-care teaching, and helps maintain a safe environment. The nurse also assists the physician and RN in complex nursing situations, follows universal precautions, and follows emergency procedures when needed.

Requirements

One year of experience as an LPN in a home care setting.
Current state LPN license.
Graduate of an accredited practical nursing program.
Proof of current CPR certification.
Proof of a current negative TB test.
Ability to work a flexible schedule and travel locally.
Excellent interpersonal skills.
Valid driver’s license and reliable transportation.

Responsibilities

Participate in planning and coordinating total patient care with the RN and according to the physician’s plan of care, and follow each patient’s nursing care plan.
Accurately report and record the patient’s condition and care, including signs and symptoms that may indicate a change.
Prepare clinical or progress notes and summaries.
Assist patients with activities of daily living and teach appropriate self-care techniques.
Provide and maintain a safe environment for the patient.
Assist the physician and Registered Nurse in complex nursing situations.
Follow universal precautions when providing patient care.
Follow emergency procedures in the event of a catastrophic event, incident, accident, injury, or significant change in the patient’s condition.
Provide daily skilled visits, including insulin administration and weekly medication setup.
