About The Position

This is a new vacancy. Signing Bonus Offered! In this community nursing role, as a General Visiting Registered Practical Nurse (RPN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. This is a Part-Time Evening line, 3pm to 11pm, includes every third weekend.

Requirements

Registration with the CNO as a Registered Practical Nurse in good standing
Valid driver’s license and access to a vehicle is required
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Passion for making a difference in your community
Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs

Nice To Haves

Experience in delivering community care is an asset

Responsibilities

Using appropriate assessment tools/techniques to assess patients’ individualized needs
Executing and continuously monitoring care plans
Engaging patients using a collaborative approach by building rapport and trust
Maintaining and update professional patient records related to patient encounters and plans
