About The Position

This is a replacement vacancy. In this role, as a Clinic Nurse (RPN) you will have scheduled appointments to care for patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. This is a 1-year maternity leave contract, shifts are from 8am to 8pm on weekdays and alternating weekends at our Cambridge Clinic.

Requirements

• Registration with the CNO as a Registered Practical Nurse in good standing
• Valid driver’s license and access to a vehicle is required
• Reliability and flexibility
• Willingness to travel to and from patient residences within a designated area
• Passion for making a difference in your community

Responsibilities

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitoring care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and update professional patient records related to patient encounters and plans
• Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
