About The Position

This is a full-time position for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in an Adult Day Program. The role involves providing direct medical care, supervision of nursing staff and participants, and ensuring compliance with regulations. The position requires a strong understanding of elder care, multidisciplinary team collaboration, and administrative responsibilities related to participant care and program operations.

Requirements

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in Massachusetts.
  • Minimum of two years’ experience working with elders and adults with physical or cognitive impairments.
  • Sensitive to the needs of elders of all socioeconomic backgrounds.
  • Ability to work cooperatively as a multidisciplinary team leader.
  • Knowledge of good body mechanics is required.

Nice To Haves

  • Some supervisory experience with other nurses and direct care staff preferred.

Responsibilities

  • Evaluate prospective participants for admission and assist in obtaining and reviewing medical information.
  • Fill out necessary documentation requiring RN license for Initial Mass Health Authorization process.
  • Supervise the admission of new participants, including physical assessments, documentation, and initiating plans for medication administration.
  • Develop, implement, coordinate, and evaluate the nursing components of a multi-disciplinary care plan for each participant.
  • Complete yearly assessments and renew care plans yearly in collaboration with the clinical team.
  • Generate and supervise other nurses in generating comprehensive MD orders and PCP renewals.
  • Maintain all medications, med documentation, administration, and glucose monitoring in compliance with DPH and Mass Health regulations.
  • Supervise the writing of monthly nursing notes, emergent/ongoing issue notes, and incident reports.
  • Provide direct medical care and supervise direct care by other nurses, including assessment of emergent issues, individual counseling, medication administration, and vital sign monitoring.
  • Coordinate care with families, community agencies, case managers, HHAs, VNAs, PTs, PCPs, MD specialists, and the clinical team.
  • Supervise Personal Assistants (PAs) and assist participants with Activities of Daily Living (ADLs), including toileting, incontinence care, ambulation, transfer, and eating.
  • Work with the Kitchen Coordinator to ensure meals meet dietary requirements.
  • Oversee all documentation and requirements related to nursing for Mass Health and SCO authorizations.
  • Work with Compliance to train and supervise on OSHA regulations and other health and safety standards.
  • Coordinate with other disciplines to provide training and orientation for staff on health issues, body mechanics, personal care, therapeutic communication, and behavioral issues.
  • Maintain a program of Nursing Competencies and the Nursing portion of PA Competencies, including initial training and yearly reviews.
  • Supervise nursing and medical students.
  • Review nursing time sheets and submit them for approval.
  • Attend all relevant meetings, including clinical, Operations, Compliance, Regulations, MADSA, and Nursing Management Role meetings.
  • Monitor medical supplies and order as needed.
  • Collaborate with the clinical team, Program Director, Admissions Coordinator, CEO, CAO, and Funding and Communications personnel for client care or ADHP Operations issues.
  • Agree to work as assigned, potentially outside regularly scheduled hours/days with advance notice when possible.

Benefits

  • Compensation: LPNs $32/hr- $37/hour DOE, RNs: $34-$42/hour DOE
