About The Position
The Program Nurse - LPN will coordinate the delivery of nursing and social services for adult day participants. This is a part-time position requiring 22.5 hours per week, spread over 3 days. The role involves conducting new client intakes and assessments, and assessing the nutritional, medical, physical, and mental functional needs of clients.
Requirements
- Graduate of accredited nursing school
- 2-3 years of nursing experience.
- State of Connecticut Nursing License
- CPR certification
- Ability to travel to client homes and other community sites as necessary related to networking and intakes.
- Availability of personal vehicle for work related travel (mileage reimbursed).
- Utilizes Microsoft suite, including outlook, and excel; flexibility and initiative to adapt to changing software technology.
Nice To Haves
- Experience working with the elderly and those with dementia/Alzheimer's Disease
- Work in a community-based setting
- Knowledge of services and entitlements for frail elderly are a plus.
Responsibilities
- Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
- Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
- All other related duties as assigned.
