About The Position

Alanté Health is seeking a Nurse Practitioner to provide comprehensive home-based primary care and telehealth services for seniors. The organization provides Chronic, Principal, Transitional, and Wellness Care Management to improve patient outcomes and reduce hospital readmissions and healthcare costs. Patients may be seen in their homes, assisted living facilities, or skilled nursing facilities. The role is described as hybrid and offers a manageable caseload of approximately 16–20 patients weekly. Flexible employment models are available, including full-time salaried, part-time, and 1099 independent contract arrangements. Profit share is available after 90 days.
