About The Position

Alamo Hospice is seeking a PRN Nurse Practitioner to serve the Waco, Texas area and surrounding counties. The role provides hospice care through home visits and is scheduled as needed, with no on-call responsibilities. The Nurse Practitioner works with a collaborative interdisciplinary hospice team to assess patient eligibility and needs, coordinate care, and update plans of care. The position offers competitive pay and flexible hours.

Requirements

Graduate from an accredited registered nursing program.
Licensed as a Nurse Practitioner in the state of practice.
At least two years of varied work experience as a professional nurse.
Current CPR certification.
Demonstrated skill in nursing practices and clinical judgment.
Ability to make home visits and provide nursing care in the home environment.
Ability to cope with family emotional stress and tolerate individual lifestyles.
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
Valid driver’s license and proof of insurance.

Responsibilities

Determine primary patient eligibility for care based on state and federal guidelines.
Assess physical, psychological, social, and spiritual factors that may affect patient and family needs, and initiate interventions with other interdisciplinary team members.
Communicate with attending physicians, interdisciplinary staff, and other agencies regarding admission or recertification and coordination of optimal care.
Use resources effectively to help meet patient needs.
Make regular visits to reassess patient eligibility and needs, and update the plan of care.
Provide nursing care in the home environment.
