About The Position

Ambercare Hospice is seeking a compassionate and dedicated PRN Nurse Practitioner to join its team in Las Cruces, New Mexico. The role provides clinical care and support to hospice patients and their families, with a focus on comfort, dignity, and quality of life. The position offers flexibility to coordinate patient visits, manage caseload priorities, and provide coverage as needed. The Nurse Practitioner works with patients in the home environment and collaborates with attending physicians, interdisciplinary staff, and other agencies to coordinate care.

Requirements

• Graduate of an accredited registered nursing program.
• Licensed as a Nurse Practitioner in the state of practice.
• At least two years of varied work experience as a professional nurse.
• Current CPR certification.
• Demonstrated nursing practice skills and clinical judgment.
• Ability to make home visits and provide nursing care in the home environment.
• Ability to cope with family emotional stress and tolerate individual lifestyles.
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
• Valid driver’s license and proof of insurance.

Responsibilities

• Determine patient eligibility for care based on state and federal guidelines.
• Gather information about physical, psychological, social, and spiritual factors affecting patient and family needs, and initiate interventions by other interdisciplinary team members.
• Communicate with attending physicians, interdisciplinary staff, and other agencies regarding admission or recertification and coordination of optimal care.
• Use resources effectively to help meet patient needs.
• Make regular visits to reassess patient eligibility and needs and update the plan of care.
