About The Position

Marin City Health and Wellness Center (MCHWC) is seeking an experienced and dynamic Nurse Practitioner to support its clinics. This role focuses on providing comprehensive family medical care, working closely with the Chief Medical Officer (CMO). The Nurse Practitioner will be responsible for delivering high-quality primary care, fostering collaborative teams, and developing clinical services tailored to the needs of the patient community. The ideal candidate will have a strong commitment to working with vulnerable populations and prior experience in primary care and leadership. MCHWC is a Federally Qualified Health Center (FQHC) dedicated to providing innovative health and wellness services with a focus on achieving health equity for African Americans. With clinic sites in Marin County and San Francisco, and a new site in San Rafael, MCHWC offers comprehensive services including medical, dental, behavioral health, substance use, and homeless services for children, adults, and seniors. MCHWC is also an eligible participant in the National Health Service Corps Loan Repayment Program.

Requirements

  • Current California license, including furnishing license (for CNM, FNP, WHNP).
  • Past work experience in leadership roles or positions.
  • Demonstrated medical clinical skills.
  • Interest in working with a diverse patient population in a community-based organization and sensitivity to their needs and health problems.
  • Ability to communicate effectively and work with individuals from a wide range of ages, socio-economic, ethnic, racial backgrounds, and disabilities.
  • Ability to work well independently under minimum supervision.
  • Valid California driver’s license, automobile, and automobile liability insurance.

Nice To Haves

  • Spanish language ability.
  • Experience with electronic health records, specifically Epic and/or eCW.
  • Community health clinic background.
  • Experience as part of a team-based care model.

Responsibilities

  • Develops and maintains clinical programs to deliver high-quality health care to patients.
  • Provides input on the impact of clinic programs and policies on patients and staff in coordination with clinical and operational leadership.
  • Maintains existing and establishes new partnerships with community partners serving patients and their families.
  • Maintains a working partnership with Marin Health and UCSF, as well as other healthcare partner organizations.
  • Assists in the development of patient care policies, protocols, and procedures related to women’s health services.
  • Contributes to the implementation of a comprehensive quality management program for women’s health services, including medical audits, protocol reviews, peer reviews, and tracking clinical measures, in cooperation with the Chief Medical Officer, Chief Operating Officer, and Quality and Risk Manager.
  • Participates in provider and staff recruitment and retention efforts.
  • Provides direct patient care with a standard of excellence appropriate for a clinical leader.
  • Provides patient follow-up and referrals as needed.
  • Provides health education and counseling to patients in a sensitive and supportive manner.
