CareBridge Health is seeking Nurse Practitioners licensed in Texas, Iowa, or Kansas, who also hold an active RN Compact license. This is a 100% virtual role, offering maximum flexibility and autonomy, with the exception of required in-person training sessions. The Nurse Practitioner will collaborate with company physicians, the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values. They will provide clinical and non-clinical support to patients, delivering urgent health care via telephone and tele-video modalities to patients receiving home and community-based services through state Medicaid programs, dual eligible members, and other assigned members. The role involves developing and implementing clinical plans of care for adult patients with chronic and complex conditions, gathering history and physical exam data, developing treatment plans, identifying and closing gaps in care, and addressing patients' and families' physical and psychosocial needs. The Nurse Practitioner will also educate patients and families, maintain contact with clinical team members and other medical providers for care coordination, maintain patient medical records, and participate in continuing education. They will also prescribe medication as permitted by state prescribing authority.
