About The Position
Join Agape Care Group as a hospice nurse practitioner and provide diagnosis, treatment, and care under the supervision of a physician, with a focus on palliative care. The role helps patients and families through their health care journeys with compassion and supports meaningful patient experiences for people facing life-limiting illnesses. Nurse practitioners have access to a supportive leadership team and professional development opportunities, with room for advancement. Pay is determined by years of experience and location.
Requirements
- A heart to serve patients and families and a passion for providing the best possible care.
- Graduate of an accredited master's, post-master's, or doctoral nursing program with formal preparation as a Nurse Practitioner.
- Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner.
- Current prescriptive authority with full DEA privileges.
- Achieve and maintain national certification in the Nurse Practitioner specialty.
- Two years of experience within the last five years in chronic disease management or long-term care.
- Reliable transportation.
- Ability to sit, stand, bend, and move intermittently, and lift at least 80–100 lbs.
