About The Position
Crossover Health is a national, team-based medical group focused on wellbeing and prevention. Its Primary Health model integrates primary care, physical medicine, mental health, health coaching, care navigation, and other services, delivered in person, virtually, and through asynchronous messaging. The Nurse Practitioner is responsible for meeting clinical, administrative, and financial performance metrics and for implementing and delivering high-quality, patient-centered care. The role provides primary care services in a health center and, depending on state requirements, general medical care under the direction of a physician.
Requirements
- At least 5 years of clinical experience in Internal Medicine or Family Practice.
- Board certification.
- Licensed to practice in the appropriate state.
- Current Basic Life Support (BLS) certification.

Responsibilities
- Provide patient-centered, comprehensive primary care services.
- Participate in selecting suitable treatment plans for patients.
- Supervise staff to ensure the care team provides appropriate care in accordance with its duties and obligations.
- Assist other care providers and extended care teams.
- Remain continuously available for medical and technical questions from employees, patient care staff, and ancillary care providers, including after-hours coverage.
- Participate in quality improvement programs and multidisciplinary quality assurance programs.
- Collaborate with the management team as needed to improve patient care.
- Cooperate with clinical outcome reviews and measurements.
- Comply with all policies, procedures, and protocols.
- Provide general medical care and treatment to patients in the health center under a physician's direction, depending on state requirements.
- Submit individual patients' healthcare plans and goals for periodic physician review and evaluation.
- Prescribe or recommend drugs and other treatments, such as physical therapy, inhalation therapy, or related therapeutic procedures.
- Refer patients to a physician for consultation or to specialized health resources for treatment when appropriate.
- Perform other duties as assigned.
