About The Position

Humana's Primary Care Organization is a rapidly expanding provider of senior-focused, value-based care, with over 340 centers across 15 states operating under the CenterWell and Conviva brands. The organization is dedicated to transforming healthcare by prioritizing seniors. As a VSP Nurse Practitioner at CenterWell Senior Primary Care, you will join a collaborative team that includes other APP's, Pharmacists, Care Coaches, and Behavioral Health Specialists, all working together to deliver personalized, high-quality care. You will report to the Associate Medical Director and be part of a team committed to supporting and empowering clinicians. The leadership fosters an environment where clinicians can provide the highest quality care and build relationships with patients.

Requirements

  • Master's degree in nursing with board certification
  • Active, unrestricted NP license in the state of practice
  • Minimum two years' Nurse Practitioner experience
  • Commitment to improving patient experience and outcomes
  • EMR experience and adapts well to new technologies
  • Participation in Tuberculosis (TB) screening program

Nice To Haves

  • Experience in Value-Based care, Primary Care or Medicare Advantage settings working with Geriatric patients
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license or Medicaid and Medicare numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines

Responsibilities

  • Provide comprehensive Primary Care to seniors
  • Collaborate daily with interdisciplinary team
  • Document in Athena EMR
  • Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time
  • Work on-site, in an outpatient office setting
