About The Position

Humana's Primary Care Organization is a large and growing senior-focused, value-based care provider with over 340 centers across 15 states operating under the CenterWell and Conviva brands. As a VSP Nurse Practitioner at CenterWell Senior Primary Care, you will be part of a collaborative team including other APP's, Pharmacists, Care Coaches, Behavioral Health Specialists, and more, all working together to deliver personalized, high-quality care. Reporting to the Associate Medical Director, you will be part of a team dedicated to supporting and empowering clinicians. The leadership is committed to promoting an environment where you will provide the highest quality care and establish relationships with those you serve. This role offers a team-based care model, more time with patients due to a lower daily patient volume, and a supportive, inclusive culture that values teamwork, innovation, and learning.

Requirements

Master's degree in nursing with board certification
Active, unrestricted NP license in the state of practice
Minimum two years' Nurse Practitioner experience
Commitment to improving patient experience and outcomes
EMR experience and adapts well to new technologies
Participation in Tuberculosis (TB) screening program

Nice To Haves

Experience in Value-Based care, Primary Care or Medicare Advantage settings working with Geriatric patients
Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license or Medicaid and Medicare numbers
Familiarity with HEDIS quality indicators and Medicare guidelines

Responsibilities

Provide comprehensive Primary Care to seniors
Collaborate daily with interdisciplinary team
Document in Athena EMR
Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time
Work on-site, in an outpatient office setting
