About The Position

Humana's Primary Care Organization is a rapidly expanding provider of senior-focused, value-based care, operating over 340 centers across 15 states under the CenterWell and Conviva brands. As a VSP Nurse Practitioner at CenterWell Senior Primary Care, you will join a collaborative team that includes other APPs, Pharmacists, Care Coaches, and Behavioral Health Specialists, all dedicated to delivering personalized, high-quality care. Reporting to the Associate Medical Director, you will contribute to an environment that supports clinicians and empowers them to provide the highest quality care and build strong patient relationships. The role emphasizes a team-based care model, allowing for more time with patients due to a lower daily patient volume, fostering deeper relationships and more personalized care within a supportive and inclusive culture that values teamwork, innovation, and learning.

Requirements

Master's degree in nursing with board certification
Active, unrestricted NP license in the state of practice
Minimum two years' Nurse Practitioner experience
Commitment to improving patient experience and outcomes
EMR experience and adapts well to new technologies
Participation in Tuberculosis (TB) screening program

Nice To Haves

Experience in Value-Based care, Primary Care or Medicare Advantage settings working with Geriatric patients
Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license or Medicaid and Medicare numbers
Familiarity with HEDIS quality indicators and Medicare guidelines

Responsibilities

Provide comprehensive Primary Care to seniors
Collaborate daily with interdisciplinary team
Document in Athena EMR
Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time
Work on-site, in an outpatient office setting
