About The Position
Eterna Primary Care is hiring a Medical Practitioner (NP/PA) to provide geriatric and post-acute care at Diversicare of Winfield in Winfield, Alabama. The practitioner will serve as a primary medical lead for patients in skilled nursing and assisted living facilities, helping bridge the transition from acute care to recovery and long-term wellness. The role focuses on high-quality, patient-centered care intended to reduce hospital readmissions and improve quality of life. The position offers autonomy in clinical decision-making and schedule management, mentorship from physician leaders, interdisciplinary collaboration, dedicated support staff, and streamlined EMR documentation. Compensation is described as up to $170K, with a comprehensive benefits package that includes CME reimbursement. Requirements
- Master’s or doctorate from an accredited NP or PA program.
- Current state license.
- Board certification.
- Active DEA registration.
- Be independent yet collaborative, have strong time-management skills, and communicate compassionately while treating patients with respect.
Nice To Haves
- Experience in geriatrics, post-acute care, or skilled nursing facility settings is strongly preferred.
Responsibilities
- Perform comprehensive assessments and manage acute and chronic conditions in complex, medically fragile populations.
- Use telehealth and evidence-based strategies to enhance patient outcomes and improve the care experience.
- Serve as a liaison between hospitals, families, and facility leadership.
- Provide clinical guidance to nursing staff and therapy teams.
- Champion quality metrics and cost-effective medicine that prioritizes patient goals.
