Requirements
- Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation; company bills on your behalf.
- 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.
- Comfortable conducting telehealth visits and navigating virtual care platforms.
- After hours availability (daily 5PM - 11PM).
- Thrives in a dynamic, evolving startup environment.
- English required
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
- Spanish preferred.
Responsibilities
- Cover weekend tele visits for home health patients in the SoCal/OC region.
- Conduct remote assessments daily 5PM to 11PM.
- Act on Circadia platform alerts.
- Ensure continuity of care for high-risk patients between weekday care team touchpoints.
- Conduct follow-up and urgent telehealth assessments on flagged patients.
- Adjust plans of care as needed.
- Coordinate with on-call providers and care teams.
- Interpret trend-based insights from the Circadia platform.
- Prioritize high-risk patients and intervene on weekend alerts to prevent unnecessary ED visits or hospitalizations.
- Complete documentation/charts same day via AI-powered tools.
- Efficiently complete tele visits for designated home health patients in the SoCal/OC area on after hours, focusing on Circadia alerts and high-acuity flagged patients.
- Surface insights to improve virtual workflows and tech integration in a home health setting.
