Requirements

Graduate of an accredited Diploma, Associate, Baccalaureate, or Masters Degree in Nursing
Current State License as a Registered Nurse
Minimum of one year nursing experience in a clinical care setting
Demonstrates clinical nursing competencies in accordance with accepted standards of practice
Able to assess and respond appropriately to the physical, emotional, and psychosocial needs of patients, families, and caregivers
Exhibits strong interpersonal communication skills, emotional intelligence, and the ability to support patients and families facing end-of-life issues
Capable of managing multiple priorities, documentation requirements, and time-sensitive responsibilities efficiently
Reliable transportation and valid automobile insurance coverage

Nice To Haves

Bachelor of Science in Nursing (BSN) preferred
Hospice or home health experience preferred

Responsibilities

Supporting patient care needs after-hours, including urgent admissions, symptom management, and other patient needs. Think of a triage/emergency room nurse for hospice patients’ after-hours in homes, facilities, and residential care communities in your neighborhood.
Providing positive, supportive communication to physicians, NPs, our hospice case managers, patients, and families.
