This role involves providing skilled nursing care to patients at the end of life (EOL), conducting visits in facilities and/or at patients' homes. The Hospice Nurse will provide on-call services after hours and complete scheduled skilled visits. Responsibilities include responding to phone calls from patients and families regarding concerns about symptoms, changes in conditions, or emergencies. The nurse will conduct home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis. This role also involves assisting the RN Case Manager (RNCM) in carrying out the plan of care, performing specialized procedures as directed, and actively participating in interdisciplinary group (IDG) collaboration to ensure the patient plan of care is current. Additionally, the nurse will assist in reinforcing patient and family education regarding end-of-life care, assume responsibility for personal and vocational growth, keep the RNCM informed of changes, and participate in self-development through further education and professional activities. The role also includes assisting with emergent visits such as continuous care (CC) and companion care as needed, and performing other duties as assigned.
