About The Position
Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The Nurse Practitioner provides patient and family education and care in a hospice setting, contributes to the interdisciplinary group’s plan of care, collaborates with physicians, monitors patient needs and responses to care, and maintains timely documentation and compliance with applicable requirements. The position is PRN.
Requirements
- Graduate of a Baccalaureate School of Nursing.
- Current state license as a Registered Nurse and as an Advanced Practice Nurse (APN).
- Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.
- Two years of experience as an APN in a clinical care setting.
- Comprehensive knowledge of pharmacology, palliative care, and end-of-life care.
Nice To Haves
- Experience in hospice or home health is preferred.
Responsibilities
- Consult with and educate patients and families about the disease process, self-care techniques, and end-of-life care.
- Consult with and assist the interdisciplinary group (IDG) in developing patient and family plans of care to assure quality care.
- Interact with attending and consulting physicians as necessary regarding palliative care, in collaboration with the hospice physician.
- Inform physicians and other personnel of changes in patient needs and outcomes of interventions.
- Evaluate patient and family responses to care.
- Submit all documentation in a timely manner.
- Perform other duties as required.
- Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
