About The Position
Frontier Hospice is hiring a per diem Hospice Nurse Practitioner to provide patient-centered hospice care in and around the Edmond area. The role involves assessing patients, diagnosing and managing medical conditions, and developing personalized care plans. The Nurse Practitioner works with patients and families, a collaborating physician, attending physicians, the hospice medical director, and the interdisciplinary team to support patients’ medical, psychological, spiritual, and other care needs. Actions are performed under the supervision of a collaborating physician and in compliance with the state Nurse Practice Act. Per diem Nurse Practitioners are paid $125 per visit.
Requirements
- Master's degree.
- Current, unencumbered Registered Nurse and Nurse Practitioner licenses in the state of practice.
- Excellent verbal and written communication abilities, including the ability to articulate complex problems.
- Computer proficiency, including typing and use of various programs, with an EMR background.
- CPR certification if required by the state.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Nice To Haves
- At least one year of hospice or palliative care experience as an RN or NP is preferred.
- Certification in a specialist area is preferred, such as Hospice and Palliative Nursing (CHPN), pediatrics, or geriatrics.
Responsibilities
- Provide comprehensive medical care, palliate and manage terminal illnesses, and comply with the state Nurse Practice Act under the supervision of a collaborating physician.
- Consult with attending physicians, the hospice medical director, and other healthcare practitioners to assess hospice eligibility and coordinate ongoing care and medical management related to terminal conditions.
- Review clinical information and coordinate and oversee patient care with the hospice interdisciplinary team and other healthcare practitioners.
- Educate and counsel patients and families on preventative care, medical problems, psychological issues, and spiritual concerns.
- Work with the interdisciplinary team to identify care goals and meet the needs of patients and their families or caregivers.
- Recommend to the Hospice Medical Director whether patients are eligible for initial hospice certification and recertification.
- Maintain current knowledge of Medicare/Medicaid, state, and federal hospice rules and regulations, and ensure compliance.
