Nurse Practitioner (NP) Hospice
About The Position
Agape Care Group is seeking a hospice and palliative care nurse practitioner to care for seriously ill patients, alleviate burdensome symptoms, and help align medical treatments with patients’ goals and values. The nurse practitioner collaborates with clinicians and teammates to provide care wherever patients call home. As an active member of the medical community in the assigned territory, the role provides ongoing palliative care, hospice services, and education. The position offers supportive leadership, professional development opportunities, and room for advancement. Agape Care Group is a regional hospice and palliative care provider serving patients through care providers in South Carolina, Georgia, North Carolina, Alabama, Kansas, Louisiana, Missouri, Oklahoma, and Virginia. Its mission is to provide comfort and support through compassionate care and meaningful experiences for patients with life-limiting illnesses and their families.
Requirements
- A heart to serve patients and families and a passion for providing the best possible care.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing preferred.
- Two years of experience within the last five years in chronic disease management or long-term care.
- Reliable transportation.
- Ability to sit, stand, bend, and move intermittently, lift at least 80–100 lbs, and effectively bear the weight of an average adult.
