Nurse Practitioner - Part-Time (OH License required)

Greater Good Health•Cleveland, OH

About The Position

Greater Good Health is hiring a part-time Nurse Practitioner to provide care to its I-SNP population in the Cleveland, Ohio area and surrounding communities. The High Risk program serves patients with significant health needs and complex conditions, aiming to prevent acute-care readmissions and overutilization of inpatient care. The Nurse Practitioner provides in-facility, in-home, and virtual clinical support, including care for patients returning home after an acute inpatient admission. The role addresses clinical and psychosocial factors that contribute to recurring healthcare utilization and allows clinicians time to address patients’ needs and concerns. The schedule is one day per week, 8 a.m.–5 p.m., with flexibility to adjust to patient and partner needs. The work includes in-clinic and in-home visits, primarily within a 30-mile service area, as well as telehealth appointments. Coverage may be needed across regions, including Cincinnati and Cleveland. The Nurse Practitioner works with patient care teams and an interdisciplinary team to coordinate holistic, high-touch care. Greater Good Health describes its environment as purpose-driven, collaborative, and supportive, with a focus on quality, outcomes, and improving access to care for older adults in underserved communities.
