About The Position

WellBe Senior Medical is seeking a Nurse Practitioner to deliver value-based primary care directly in patients’ homes in Alpharetta, Georgia. The role emphasizes autonomous, outcomes-based care, addressing patients’ social determinants of health as well as their medical needs. The Nurse Practitioner manages a dedicated patient panel with support from an interdisciplinary care team, including daily team check-ins, ongoing training, and one-to-one mentorship.

The position involves comprehensive primary care visits and physical examinations in patients’ homes, longitudinal care planning for high-risk patients, coordination of care transitions, and treatment of acute needs at home to help prevent emergency room visits. Compensation is $104,763–$166,750, with mileage and full benefits. The posting also offers a $10,000 sign-on bonus and says relocation is available.

Requirements

• Master’s degree as a Nurse Practitioner.
• Active, unencumbered Nurse Practitioner license in Georgia.
• Board certification through ANCC or AANP.
• Valid driver’s license and a vehicle for local field-based travel.
• Must be legally authorized to work in the United States without employer visa sponsorship now or in the future.
• Must successfully complete a pre-employment drug screening; ongoing employment may be contingent on compliance with the Drug-Free Workplace Policy and applicable drug testing.
• Employment is contingent upon successful completion of a background check, as permitted by law, and satisfactory monthly FACIS checks.
