About The Position
Community Health Network is seeking an experienced, family medicine-certified advanced practice provider for an Employer Health Onsite primary care role at Warren Township Health and Wellness in Indianapolis, Indiana. The position focuses on the well-being of employees and their beneficiaries. A minimum of two years of experience as an advanced practice provider is required. Requirements
- Family medicine certification as an advanced practice provider.

Responsibilities
