About The Position
Community Health Network is seeking an experienced, family medicine-certified advanced practice provider for an Employer Health Onsite primary care role at Warren Township Health and Wellness in Indianapolis, Indiana. The role focuses on the well-being of employees and their beneficiaries and is part of Community Health Network, an integrated healthcare system with more than 1,300 providers. The position uses the EPIC electronic medical record. No holidays are required.
Requirements
- At least two years of experience as an advanced practice provider.
- Family medicine certification.
- Educational background consistent with the requirements for licensure and certification in Indiana.
Responsibilities
- Provide routine check-ups and preventive care.
- Diagnose and treat illnesses or injuries.
- Manage chronic or complex health conditions.
- Manage medications.
- Refer patients to specialists for more serious conditions.
